Requirements: To best assist you, the following marketing materials are necessary.

All Models Represented Must Have:

1. (100) Composite Cards with layout
2. (6) 9x12 Portfolio Prints Professionally Retouched
3. Excellent Photos Approved by Vee Muka and/or Photo Shoot

High-Fashion/Runway/Editorial Models Must Have:

1. Professional Runway Skills
2. Strong Portfolio Book

Commercial/Print/TV Film Models Must Have:

1. TV/Film Composite Card Resume with V-Model Layout
2. Personalized, Professional DVD Bio For Casting Directors

Kids/Plus/Sophisticate Must Have:

1. TV/Film Composite Card Resume with V-Model Layout and/or Composite Cards with V-Models Layout

Packages: V-Models will refer Photo/Printing packages of necessary marketing materials.

1. (100) 9x8 Composite Cards with V-Models Layout plus (6) 9x12s Portfolio Prints Professionally Retouched. The photos will be selected from a CD provided by the model. The CD will have no more than 10 photos in high resolution (300 dpi). $175 subject to change.
2. Exclusive Professional Photo Shoot (models must be selected by Vee Muka). This package includes all of the above plus an opportunity to build and strengthen portfolios. $950 subject to change.
3. 9x12 Black and Blue V-Models Portfolio Book. $100 each subject to change.
4. For models selected by Vee Muka to be under Exclusive Contract, V-Models will invest in all of the above marketing materials.
5. Any Clients, Photographers, and Printing that we give business to we get 40% of total cost.

I acknowledge that I am at least 18 years of age (if under 18, parent or guardian must sign form) and that I am, in good faith, seeking to become affiliated with V-models to use my name (or any fictional names) and photographs in all forms and media in all matters, which are legal and lawful and deemed in good taste by V-models. I waive the right to approve the finished result of the use including written copy that may be connected therewith.

Responsibility Release Form

I acknowledge that I am an invitee of V-models in its offices, hotels or conference rooms or other premises for purposes of modeling, model search or any other related purposes. In that connection I agree to assume full responsibility for any injuries which might occur to me in or about such premises, including without limitations any claim for personal injuries arising out of the negligence of V-models or its offices, employees and agents.

The marketing materials, listed above, are required by our clients. They are designed to enhance your opportunities, as well as prepare you for the repetitive rejections, which are inevitable in this industry. All marketing materials of a model, regardless of origin, shall become the exclusive property of V-Models and shall remain in the office at all times, once tendered. It is the responsibility of the model to provide V-Models with up-to-date contact information and to continue reasonably regular communication with V-Models. If the model does not contact V-Models for more than 20 consecutive days, the model shall forfeit all rights under this agreement, including any claim to previous payments or marketing materials. All models must provide proper identification. Models that are minors, must be accompanied by a guardian.
